Every patient seen at Complete Chiropractic will be asked to fill out a Health History and Patient Health Information Consent Form.


Patients being seen for an injury related to an auto accident will be asked to also complete our Auto Accident Questionaire.


Patients needing their health information released to Complete Chiropractic or from Complete Chiropractic to a third party will need to fill out a Patient Health Information Release Form.
